If you are unable to complete and submit the online order form, please use the following method of submitting your application:
- Download the application form (choose a sample according to the topic of your request)
- Send the completed application to emaildzi@dzi.gov.ua with additional documents (if necessary)
- Wait for feedback.
Please note that the applications indicate the deadlines for completing the work in working days, starting the day after payment of the invoice for the relevant work.
